Director of Nursing (DON)

Position Summary
The Director of Nursing is accountable for the development and maintenance of nursing service objective and standards of nursing practice. Has administrative authority, responsibility and accountability for functions, activities, and training of nursing staff. Is responsible for the planning, development, and implementation of care delivery systems. Orients, instructs, and supervises personnel and functions. Coordinates nursing services with other departments. Maintains supplies and adequate equipment. Maintains compliance with Federal, State, and local standards and regulations to assure quality outcome of care.
Essential Responsibilities and Duties
Responsibility: To Facilitate the Care of the Resident Through a Well-Defined Care Delivery System to Assure Quality Outcome of Care
- Oversees the entire RAI process including electronic submission, assessment reference date and lock date.
- Coordinates MDS and care planning process. Will develop plan of care in cooperation with nursing staff and interdisciplinary care plan team.
- Serves as consultant on admitting and discharge policies and procedures in cooperation with Social Services and Administration and completes referrals in a timely manner. Attends or conducts patient care conferences as needed.
- Maintains total nursing care of patients and the accuracy of the nursing care records thru management of nursing staff.
- Implements procedures for general nursing care and for aseptic techniques by applying nursing policies and procedures.
- Maintains updated written nursing procedure manuals at each nursing station, in cooperation with education directors.
- Coordinates nursing services for the resident in the facility with other ancillary services and interdisciplinary team members.
- Accompanies or assigns other qualified nursing personnel to accompany physicians when attending patients.
RESPONSIBILITY: Has Knowledge of and Consistently Applies Established Nursing Policies and Procedures According to State and Federal Regulations and Nurse Practice Act.

- Implement a medication administration procedure that assures accurate, timely medicine to all residents.
- Meets with the pharmaceutical consultant routinely to assure proper medication disposition and follow-up recommendations for drug interactions.
- Completes forms properly and timely as required by State and Federal agencies; i.e., QAR, MDH, etc.

Qualifications
Education and Experience
- Must be a Registered Nurse, currently licensed by the State of Minnesota, who has training in rehabilitation nursing, gerontology, nursing service administration, management, supervision and psychiatric or geriatric nursing before or within the first twelve (12) months after appointment as Director of Nursing Services.
- Must possess the ability to communicate verbally and in writing when directing the care of residents.

Assistant Director of Nursing (ADON)

Position Summary
The ADON is accountable for assisting the DON in directing the activities of licensed and non-licensed personnel to ensure the delivery of quality care to the residents. The ADON oversees, facilitates, and manages occupational health and staff development functions within the facility alongside and in the absence of a Clinical Educator. The occupational health element of this position includes infection control, coordinating all elements of the employee injury management process, health care worker health programs and resident/employee safety. The ADON is responsible for surveillance, maintaining records, data collection, and assessment, evaluation of Incident Reports, reporting to Safety Committee, Infection Control Committee, and Quality Assessment/Assurance Committee. The ADON also facilitates staff development through orientation, hands-on training, competency evaluations, preceptorships, and audits. The ADON promotes quality care through the development of knowledge, skills and aptitude of staff.
Essential Responsibilities and Duties
Responsibilities: Resident Care
- This position is responsible for assisting the Director of Nursing in planning, developing, and supervising the activities of licensed and non-licensed personnel who provide health care and nursing services to the nursing home residents.

Plans for and facilitates professional, management, leadership, and personal growth opportunities. - Participates in staff competency evaluations and performance audits, and provides hands on, just-in-time training to appropriately respond to facility education needs.
- Facilitates any requested/required in-house education sessions or department meetings as directed by the Facility Administrator.
- Oversees student nurse/nursing assistant schedules for clinical experiences in the facility.
RESPONSIBILITY: Compliance with Injury Management Program
- Promotes overall safety of employees and residents, collects and assesses safety and infection control data.
- Coordinates and oversees the Employee-Right-To-Know Program and the AWAIR Program.

Certified Nursing Assistant (CNA)

Position Summary
The CNA is responsible for providing residents with the highest quality of direct routine care in all activities of daily living, under the supervision of the LPN or RN and in accordance with the MDH Nursing Assistant Program standards, the Resident’s Bill of Rights and the facility Quality Assurance and Performance Improvement (QAPI) program. According to the Plan of Care for the resident, CNAs apply skills in basic nursing, personal care and basic restorative services while understanding the mental health and social service needs of the resident and being mindful of the needs in cognitively impaired residents. The CNA is part of the interdisciplinary care team that is dedicated to promoting independence, autonomous decision-making and facilitating residents to achieve and maintain the highest practicable physical, mental and psychosocial well-being.
Essential Responsibilities and Duties
Quality of Care
- Provides ongoing resident care upholding and promoting the resident’s right to be free from abuse, mistreatment, neglect or misappropriation of resident property, immediately reporting any suspicions, reports or witnessed events to appropriate facility staff.
- Forms quality relationships, communicating and interacting competently on a one-to-one basis while integrating the resident-centered Plan of Care.
- Demonstrates sensitivity to physical, mental and psychosocial needs as well as maintaining dignity, while skillfully performing directed nursing, personal cares and restorative services.

Basic Nursing, Personal Cares And Basic Restorative Services
- Facilitates Plan of Care through completion of nursing responsibilities, clearly communicating resident needs to primary caregivers and reporting changes and responses to interventions to the LPN or RN.
- Completes basic nursing cares as directed such as taking and recording vital signs; measuring and recording height and weight; recognizing abnormal changes in body functioning and reporting to the LPN or RN; and caring for residents who are actively dying.
- Completes personal cares as directed such as bathing; grooming; mouth care; dressing; toileting; eating and hydration; proper feeding techniques; skin care; and transfers, positioning and turning.
- Facilitates adequate nutritional intake for resident through assisting to and from the dining area; feeding or assisting to feed; monitoring and recording intake and output; passing water and offering fluids; passing and collecting trays; serving supplemental nourishment as directed by nurse; maintaining awareness of dietary restrictions; and following safety guidelines including wearing gloves when handling food or hydration.
- Completes basic restorative services as directed such as training residents in self-care and according to the resident’s abilities; use of assistive devices in transferring, ambulation, eating, and dressing; maintenance of range of motion; proper turning and positioning in bed and chair; bowel and bladder training; and care and use of prosthetic and orthotic devices.
- Assists nurse with special procedures as directed, including implementing and completing critical care sheets, participating in care conferences, etc.
- Provides and ensures nursing care activities are practiced in accordance with the MDH Nursing Assistant standards while protecting residents’ dignity, privacy and when possible autonomous decision-making.
Facilitates assistance in areas such as getting to and participating in activities as well as assistance in resolving any grievances or disputes. - Articulates meaningful discussion with residents based on their unique backgrounds, likes and dislikes.
- Maintains care and security of residents’ personal possessions.
- Interacts with resident’s family, support network, visitors and members of the interdisciplinary care team in a courteous and cooperative manner.

- Strictly follows Standard Precaution infection control procedures and OSHA guidelines regarding bloodborne pathogens to provide safe nursing care to include proper handwashing; maintaining disinfection/cleaning schedules; wearing PPE such as gloves, face shield, fluid resistant gowns, etc.
- Maintains a clean environment in resident rooms and facility following infection control procedures to bag infectious linens and equipment or proper disposal of them.
- Avoids the need for restraints in accordance with DHS nursing home standards.
- Ensures all cares are documented and that resident records are complete through systematic collection, reporting and documentation of data that is secure from unauthorized access and follows state and federal regulations, including HIPAA to ensure privacy and maintain confidentiality.
- Promptly reports and documents incidents and errors involving residents, staff, visitors or self.
- Prioritizes work assignments consistent with number and level of staff available and resident care acuity.
- Reviews departing staff report at start of shift, ensures duties and delegated tasks are complete at end of shift and reports off to nurse when leaving.

LPN Care Coordinator

Position Summary
The LPN Care Coordinator is accountable for assisting the Director of Nursing/Assistant Director of Nursing and Nurse Manager, RN in the management and leadership of a specific nursing unit. This position, under the direction of the Director of Nursing and RN Nurse Manager, assists in the evaluation of available resources to assure quality outcomes of care. They also assist in the direction and implementation of the nursing process. This includes assisting the Director of Nursing and RN Nurse Manager with systematic data collection; assist with the development of care plan, and implementation of care plan and revision of plan. The Care Coordinator, LPN coordinates the management of the care of residents through a well-defined care delivery system.
Responsibility: To assist in the development of the Comprehensive Care Planning Process under the Direction of the RN
- Systematically collects data about residents through interview of resident and family, observation, discussion with physician and review of medical records.
- Gather data to assist with the assessment of residents and applies knowledge to facilitate the therapeutic plan of care.
- Assists and is knowledgeable in the development of the MDS, RAPS and care plan in collaboration with the interdisciplinary team.
- Assists in the implementation of the care plan by monitoring responsibility, developing communication systems and following up on outcomes.
- Attends quarterly and annual care conferences under the direction of the RN, meeting the needs of the resident and family.
- Monitors for changes of resident’s conditions, reports the changes to an RN, and revises the plan of care as delegated by an RN to reflect the current resident status consistent with facility protocols, and physicians’ orders.
- Assist in the data gathering for completion of assessments: Tardive Dyskenesia, restraint, falls, pain, bladder, Braden scale, smoking, etc. as delegated by the RN.
- Assists in the data collection for the completion of the functional care plans by other nursing personnel.

MDS Coordinator

Position Summary
The MDS Coordinator is accountable for overseeing the completion, editing, locking and transmission process of all MDS assessments. The overall responsibility of this position is to help assure timely completion of the MDS with a focus on accuracy, case mix maximization, and regulatory compliance. He/she oversees the clinical and regulatory component of MDS completion related to reimbursement. This position also coordinates care plan completion with the Interdisciplinary Team (IDT), and renders clinical support to nursing staff to enhance accurate assessments, care plans and excellent quality of care.
Essential Responsibilities and Duties
To assure timely completion of the MDS with a focus on accuracy, case mix maximization, and regulatory compliance
- Completes MDS in Electronic Clinical System based on observation of the resident, record review, data collection worksheets, NAR data, and interview with residents, families and staff.
- Updates resident information on Point of Care documentation system as each resident’s assessment window opens and communicate that information to all staff responsible for entering resident data into Point of Care system, either directly or through the Nurse Managers.
- Oversees timely completion of the MDS.
- Completes Resident Assessment Protocol (RAP) documentation based on regulatory requirements, MDS information obtained, and in accordance with professional standards of practice.
- Works with Interdisciplinary Team (IDT) to help assure care plans are comprehensive and completed on time.
- Submits completed MDS within specified time frames; monitors validation reports for errors or other problems; corrects errors or problems, as needed.
- Oversees completion of necessary paperwork including Medicare certification and recertification forms; required notice; demand bills (including required paperwork for medical assistance payor); clinical documentation to support Medicare coverage; Monitors for Medicare eligibility after resident is admitted, e.g. rehospitalization or 30-day window.
- Oversees eligibility for payors such as MSHO or insurance, and completion of necessary paperwork to assure coverage.
- Provides incidental direct care several times a week to monitor residents’ needs and staff interactions with the goal of increasing MDS coding accuracy.
